Please make sure you provide all these documents with your application.

• Completed application forms (enrolment and homestay)
• Completed medical form returned
• Photocopy of your Passport details and any student Visa and Permit if already obtained
• Details of your Fee Protection Insurance.
  (Or a completed application form for Fee Protection Insurance, including premium)
• Signed contract
• Previous school records and achievement certificates
• Two testimonials, one must be from your schoolteacher or Principal and their contact details
• A personal Essay, which describes your family and personal interests.

Please include the following in your essay

• Photographs of yourself with your family
• How you see yourself, your personality, your interests, your values
• Your relationship with your family and friends
• Describe your responsibilities at home and outside the family
• What do you wish to gain at Otamatea High School
• How would you make attempts to cope with the New Zealand lifestyle?

All information supplied should be correct and true. If it is discovered incorrect information is supplied, your enrolment at Otamatea High School could be at risk.
